CLASSES ARE ALWAYS OPEN TO ALL

ST. JEROME CHILDREN & YOUTH!

Religious education Classes and Sacramental Preparations are:

Tuesdays from 6:00 - 7:30 pm for grades 1-6 and

Thursdays from 6:00 - 7:30 pm for grades 7-12

Copies of these are also available in the church, by the Baptismal font

and in the church office.

Registration for Children and Youth in 1st to 12th grade

TO REGISTER, THE FOLLOWING DOCUMENTS ARE REQUIRED:

2025-2026 Registration Form - ONE for EVERY child *Llenar la forma de registro en línea o en la oficina.
2025-2026 St. Jerome Census Form - ONE for each FAMILY * Censo 2025-2026, uno por familia.
Baptism Certificate and Birth Certificate of the child - IF IT’S NOT ON FILE ALREADY * Certificado de . Nacimiento y de Bautizo.
Archdiocesan Emergency Medical Information Form - ONE for EACH child *Formulario de información Médica.

Costo de registro:

⟹One child: $65.00 ⟹ Two children: $90.00 ⟹ Three or more children: $105.00

There is an additional cost of a REQUIRED $10.00 PER CHILD UNIFORM T-SHIRT

CATHOLIC SCHOOL YOUTH & CHILDREN
Planning to celebrate the sacraments of Confirmation, Holy Communion or OCIA here at St. Jerome? You should FILL OUT A REGISTRATION FORM AND BRING
THE REQUIRED DOCUMENTS AND FEE FOR CATHOLIC SCHOOL YOUTH:
$50 + $10 for a St. Jerome Religious Education T-shirt.
